Wireless controllers do not boot from microSD card

Symptoms:

A wireless controller fails to boot from firmware on the microSD card in the \sdcard0\boot directory. Instead, the controller STAT LED flashes a series of three red blinks when the problem occurs.

The controller should boot from the card when the controller is turned off, the card is inserted, and the controller is turned back on. (Or when the card is inserted and the controller is immediately reset with the RESET button.)
